Burnout Syndrome, Are you burnt out?

The concept of Burnout syndrome came into limelight in the last 40 years. According to Webster Dictionary, Burnout Syndrome is simply “physical or emotional exhaustion, especially as a result of long term stress or dissipation”. It is a response to prolong exposure to workplace stress.

Although the concept was first propounded by Henry Fredenberger in 1974, it was later studied by latter day workers like Maslash & Jackson. Infact, one of the tools for studying Burnt Out Syndrome is called Maslash Burnout Inventory (a 22 items questionnaire).


It is a syndrome that can affect doctors although all health workers inclusive. Even though all health workers can be affected, it has been observed to affect mainly nurses and doctors. Generally it affects those that their work involves interaction with people or recipients of services which includes others like Educational, police and judiciary workers.

Who are specifically at risk among these workers?
Those who work long hours
Those who work in high emotional demanding situations
Workers where there is inadequate personals
Workers where there is lack of professional recognition
Those exposed to workplace hazards
Workers with constant contact with client sufferings, pains and even death
Those in regular conflict with co-workers
Workers with excessive work load

It is said to be on the increase globally especially in the light of changing work place- more uncertainty, instability, changing work contract and in some cases increasing work load.
It has three component- Emotional Exhaustion, depersonalization and reduced personal accomplishment. The emotional exhaustion is the commonest aspect.

        If you have any of this you may be having burn out syndrome.

 Implications of Burnt Out Syndrome
The impact is enormous on the workers and the organization they work
Excessive absenteeism
High job turnover
Inappropriate and excessive application for sick leave or sick off.
Reduced overall personal wellbeing
Intention and desire of worker to leave the job
Decrease in workers’ productivity
Poor target performance
Tendency to feel fatigue 
Inability to provide basic service to client or customers
Deterioration in social relationship with friends and family etc
Poor effectiveness at work

Even though, the syndrome is becoming more and more recognized with it associated problems, it must be noted that it is preventable.
To avoid this effect on yourself or your organization, you need an experience counselor and a life skill trainer may help.

Individual strategies which include being aware of the problem, developing interpersonal, social and team building skills may help. Other skills that would help include time management, having

a schedule for relaxation and stress relieve program.
Organizations should identify this problem and ensure efforts are made to ameliorate the effects and reduce predisposing factors among their workers.

Furthermore, workplace hazards whether physical, biological or chemical should be reduced. There should be improvement of work schedule, adoption of effective conflict resolution mechanism at workplace, adequate staffing and developing a responsive human resource department.

Differentiating doctors

There is something that have observed that is making or pushing some difference into doctors and his practice. These are different from the longer years spent in medical school, level of competence require for practice, level of trust, dedication and integrity required for practice. Also the high level of regard by the society and expectation by the society. I came up with
some.

The Media is likely to feast on one malpractice than remember millions of treatment given to people with excellent outcome, safe deliveries of pregnant women and other excellent therapeutic intervention given globally daily.

We are in a highly regulated “business”. The profession and practice are stringently regulated.
To most of the patient in ER, each is likely to believe his case is the topmost emergency. Off course doctors triage!
Taxman would always come even though in some countries doctors work is bordering on charity. No tax reduction mind you!
There are other variables trying to influence your practice and profession-the Government, politics and public opinion etc. In fact in many countries healthcare issue is a election, and in one or two coup issues! 
Once there is a protocol, you have got to reduce some creativity on such "hard facts", good it really help a lot. It certainly works, they are evidence based.
The “insurance man” and “Bean counters” are encroaching on the independence of physicians globally!

7 great attitude to survive internship year

The internship year can be grueling and appear daunting to many new doctors. However this does not have to be. The following attitude would help you survive the year happy and successful. Attitude is one of the major ingredients for success at work place aside technical competence.
The following would help a house officer excel during house job happy and successful.


Hardwork                                                                                                             
Housemanship entails a lot of work in many centre. As an intern you would now be first point of call for the team on call, even if a medical student you take call with the team on call, this is a higher level of expectation plus responsibility. There would now be ward-rounds followed by carrying out post round orders (Registrar,Senior Registrar and Consultant rounds), House officer reviews/Rounds and presentations. Off course presentation. Internship is still part of your training hence you have to bear with all those ‘stuffy sessions’!                                                                                             
Therefore it is necessary to know that imbibing virtue of hardwork would  aid your survival and excelling as an intern. A firm and discipline unit consultant would not waste time to extend to a varying period a lazy House Officer.  Be prepared for grueling long hours in a busy medical centre.

Humility                                                                                                                                
Internship years is part of training or final path of being MB;BS degree holder in many countries and first year of your residency training in some countries. You still got to learn, your university may have graduated you but the lisencing body have not yet set you free to heal the world. So with humility learn the rope. Be determine to avoid pride, you are the lowest part of the “medical food chain". A haughty house Officer is likely to have more conflict with his Registrar or Senior Registrars and if pronounced even the Consultant who have to sign you off at the completion of your rotation. Off course, it is not that you be unassertive but avoid behaviour of "now am a doctor!" and the whole world must bow. 

Be humble to know that the bulk does not stop at your table. If you don’t know be humble enough to ask your Seniors. Your will likely avoid penalty by doing that and save the patients unnecessary harm and you will also learn in the long run.

Thoroughness                                                                                                                            
Medical field is becoming more litigious. Many poorly treated patients are looking for the doctor to revenge on. Document all your necessary clinical actions, necessary patients reaction to your action e.g. avoiding medications or examinations. Off course counsel such patient. Fill necessary chart! Document all your rounds or review! Pass the bulk if necessary by informing your superior as soon as possible.     


Learning Mind set
After your induction know that, that is not the end of learning. Infact medicine is one profession that you will have to keep learning till you stop practicing! The field just keep expanding and changing! The work might be daunting. Set aside at least an hour to read cases you managed in your team. An Oxford Handbook maybe handy- it is rich, short and concise!

Empathy 
Empathy towards your patient is very important. It is therapeutic in itself. You are the closest doctor to your team’s patients. Your reaction is likely part of the patient would use to judge the managing team.

Reliability                                                                                                                             
You are part of the medical team. Even though the lowest in the ladder of the team's doctors. Your role is crucial. Your consultant would prefer an House Officer that would respond to the call from a nurse about a patient promptly and also a proactive House Officer who is competent enough at his/her level.  Know your official role and try and play it well. A good house Officer is one that can key into the team very well.

You are now part of a team as an House Officer and have responsibility  for the success or failure in some way!

Don't Get Swindled! The Swindler on the prowl!!

There appears to no end to scam mails in our in boxes. Even though some of them are out-rightly stupid, ridiculous and petty. Such mail may be a link to defrauding you.The essential tip is to be cautious. 
I recommend the following to reduce burden of such mail in your inbox.
  1. Be careful where you drop your mail address whether offline or online.. You are likely to get plenty scam mails if you have tendency to feel every e-forms you find on the the internet. Drop your email address only only on trusted site.
  2. You can block such email address. They don't come with unsubscribing link if they do it maybe further way to ensnare you.
  3. Get such mail into the spam box.
  4. Add suspicious e-mail address to blocked sender list if you mail service provider can do that.
  5. Don't carry out instruction emanating from such fraudulent mails.
See the scam mail below. If you receive such  check  the real address by placing the cursor on the name of sender there would be dis concordance. By the time i place over UNDP INFO it read grantop@outlook.com, certainly that is not UNDP official email.

The other scam mail  we will consider is below though not an offer





This was scary because the recipient owns a GT Bank account, but what gave it away was that he did not transact recently on the account, and didn't have such profile on his account and again simply placing the cursor on the sender name(DON'T CLICK) it revealed -nicky.oconell@ hotmail.co.uk.
